OUTPATIENT SERVICES:
Consumers are seen on a scheduled or emergency basis. Services
are provided to an individual with a mental illness or emotional problem or
to a person whose daily life is disrupted by personal or interpersonal
problems. Individual, family, and group therapy are provided to meet the
specific needs of each consumer. Medication management is provided by agency
psychiatrists.
CASE MANAGEMENT (ADULTS): These services are
available for individuals who have both a long-term mental illness and
difficulty adjusting to community living. Case Management services include
development of individualized service plans, making referrals to appropriate
service providers and assuring our consumer receive the services he or she
needs. Our goal in Case Management is to assist an individual with mental
illness to remain in the community, rather than experience long-term
hospitalizations.
THERAPEUTIC REHABILITION: This goal oriented
program is designed to benefit individuals with severe mental illness, who
require less than 24 hour a day care, but more than outpatient counseling.
Participants of Therapeutic Rehabilitation are involved in the treatment
planning process and are involved in activities which will foster the
development and enhancement of social and daily living skills. Ultimately,
the purpose of the program is to enable individuals to live, learn, and work
in the community.
SOCIAL CLUB DROP-IN: Personal growth, peer
support and self-help are the focus for participants of the Social Drop-In
Group. Social Drop-In Group members plan activities such as an afternoon of
bowling, a trip to the mall or a picnic in the park. Program staff is
available to assist in carrying out these plans. Membership in the program
is voluntary; however participants are usually involved in other agency
programs such as Therapeutic Rehabilitation or Outpatient Counseling.
PARTIAL HOSPITALIZATION: This daytime program
is for adolescent and preadolescents with severe emotional disorders. This
program includes an intense educational component within the school year,
daily psychiatric monitoring, and intensive therapy throughout the year.
SEVERELY EMOTIONALLY DISABLED CHILDREN’S CASE
MANAGEMENT PROGRAM: The SED Children’s Case Management Program serves
children who are having difficulty in more than one area of their lives,
such as in school, at home and in the community. This program coordinates
communication between the children, their family, and any involved
professional, such as the Department for Social Services, mental health
therapists, doctors, and school representatives. The goal of this program is
to keep the child in his or her community and to improve his or her
functioning.
HOSPITAL SERVICES: Agency staff will work with
the hospitals to facilitate voluntary patient admission to, and discharge
from, these programs. Persons who require more intensive care or longer
treatment may be hospitalized at Western State Hospital in Hopkinsville.
PREADMISSION SCREENING AND RESIDENT REVIEW (PASRR):
Four Rivers Behavioral Health will conduct a comprehensive assessment,
referred to as a PASRR Level II evaluation for individuals with severe
mental illness, mental retardation, or other related conditions in Medicaid
certified nursing facilities, who are awaiting nursing home admission, to
determine the person’s need for specialized services. The PASRR evaluation
was designed to eliminate the inappropriate placement of people with mental
illness, mental retardation and other related conditions in Medicaid
certified nursing facilities. |
